This form must be completed at least 48 hours prior to start of daycare/boarding or grooming appointment and only needs to be completed ONCE. Without this form, we may not be able to accommodate your pet.

PLEASE READ CAREFULLY. DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ AND UNDERSTAND IT IN IT'S ENTIRETY.
We reserve the right to refuse a pet at check-in for any reason including, without limitation, pet's sickness, injuries, or behaviors that could jeopardize the health or safety of other pets or our staff. If your pet has a medical condition or is on medication, please let us know so we can evaluate the pet's condition.
You acknowledge and agree that in the unlikely event your pet is injured by another pet, you release CLUB NASH, LLC and its employees from any and all liability. Please be advised that your pet will co-mingle with other dogs (if dog-friendly). Infectious diseases, minor injuries, dog bites, etc. may occur. You understand that it is possible for a pet to become ill even if vaccinated. If your pet injures another pet, you will be solely responsible for the injury to the other pet(s) as well as your own pet. You assume all risks and accept the responsibility of all costs for treatments. You further agree to hold the owners and employees of CLUB NASH, LLC harmless from expenses incurred for such treatment. If your pet destroys any property of CLUB NASH, LLC during his/her stay with us, you acknowledge and agree that you are responsible for charges incurred to replace such items.
If you do not pick up your pet at the agreed time, you hereby authorize us to continue services at your expense to be billed to you by invoice. CLUB NASH, LLC will attempt to contact you by telephone and/or email advising you that if your pet is not picked up within a reasonable time period, your pet will be delivered to a third-party adoption center, humane society, or other similar agency. You understand that you may lose ownership of your pet under these circumstances. If you fail to pick up your pet for any reason, you release CLUB NASH, LLC from all further liability and responsibility for your pet. You shall remain liable to us for all unpaid charges, including the court costs and attorney fees incurred in the collection of the charges.
We reserve the right to use photographs or videos of your pet taken during their stay, which may be used for marketing materials.
If we cannot reach you, you authorize CLUB NASH, LLC to contact your provided emergency contact. You agree that your emergency contact shall have your complete authority to make any and all decisions, including those relating to the health of your pet and the expenditure of funds, for or on the behalf of you and your pet. In cases we deem serious or necessary, your pet will be taken to the emergency vet before attempting to contact you.
This agreement will remain in effect for any current or future pets you may bring to CLUB NASH, LLC until it is updated. Your electronic signature verifies your understanding and consent to this agreement.
If your pet becomes ill or injured in what is deemed by Club Nash, LLC as a non-emergency, and we are unable to reach you or your emergency contact, please indicate your wishes:
Perform whatever services the veterinarian deems necessary for the best care of my pet until someone can be reached
Do not administer any medical services until specific authorization is given

Grooming

Owners must inform CLUB NASH LLC if their pet bites, has bitten, or is aggressive to people, other pets, or specific grooming procedures. Muzzling may be used if necessary. Muzzling will not harm your pet and protects both the pet and groomer. Club Nash LLC reserves the right to refuse/stop services for such pet at any time before or during the grooming process, and charge an Aggressive Dog Fee, in addition to the regular grooming charge.
Grooming procedures can sometimes be stressful, especially for a senior pet or a pet with health problems. Grooming can expose hidden or aggravate known medical problems, during or after the groom. You agree to give Club Nash LLC permission to obtain immediate Veterinary treatment for your pet, should it be deemed necessary. We will do our best to contact you first, then take your pet to your authorized Veterinarian or the nearest available Veterinarian. It is agreed that all expenses for Veterinary care will be covered by the pet owner upon signing this agreement.
Pets with matted coats need extra attention during their grooming session. Mats can be very difficult to remove, and may require the pet to be shaved. Removing a heavily matted coat includes risks of nicks, cuts or abrasions. After effects of mat removal can include itchiness, skin redness, self-inflicted irritations or abrasions, and failure of the hair to regrow. There is an extra charge for dematting.

If fleas or ticks are found during the grooming process, you will be charged an additional fee for removal and treatment.

 There is always a possibility that an accident could occur. Even though we use extreme caution and care in all situations, possible problems could occur including cuts, nicks and scratches.
By signing this agreement, you agree to hold Club Nash LLC and its employees harmless from any damage, loss or claim arising from any condition of your pet, either known or unknown, to Club Nash LLC. It is also further understood and agreed that the terms of this agreement can change at any time without notice.

What to pack

The only items required to bring are food, collar, and leash. We strongly advise against bringing bowls, beds, toys, etc. We are not responsible for loss or damage to personal items. All medications must be clearly labeled with the pet's name and instructions.

All food must be pre-portioned, bagged, and labeled with your pet's name.

Pick up/ drop off

Please give us a 30 minute heads up before dropping off or picking up. Upon your arrival, we will come to your vehicle to retrieve your dog and and their bag. We do this to avoid other dogs getting out and so we can introduce dogs into the pack properly. When picking up your dog, we will bring them and their belongings out to your vehicle. We also offer pick up and drop off services.

Daily schedule

Dogs are under constant supervision. We are able to keep all special feeding and potty schedules as requested by pet parents. Every dog is fed separately to avoid food aggression. We group dogs by age, size, and energy levels. Groups are alternated for play time, rest time, feeding, etc. There is a fenced in area outside for free-roaming and all dogs are allowed on the furniture indoors.

Dogs who aren't dog-friendly

We are currently able to board 1 non-social dog at a time. We have a separate area set up with access to inside and outside. They will be completely segregated from the other dogs, but still be able to have constant interaction with humans.

Kenneling/crating

Club Nash is a kennel-free facility. We do not have any kennels or cages on the property. If you request your dog to be kenneled for training purposes, please provide a crate and instructions on kenneling schedule.
